If funding is approved, a final report on the status/outcome of the Activity/Program will be requested by the Chapter, Region and Volunteer Relations Department upon the completion of the Program and/or the exhaustion of Chapter Innovation Grant Program funds.
Final reports are due within 30 days of Activity/Program to the Chapter, Region and Volunteer Relations Department.
Final reports should include all invoices and/or receipts as well as a summary outlining results of the Activity/Program following the SMART Goals format:
Specific
Specific goals have a desired outcome that is clearly understood with action to be taken to accomplish the goal.
Example: Sponsoring XYZ event at local university to recruit at least 8 student affiliates.
Measurable
A quantifiable objective so that you can track progress. Define what data will be used to measure the goal and set a method for collection.
Example: Sponsoring XYZ event at local university to recruit at least 8 student affiliates.
Measure: If students join, we can pull a Chapter Member Search report to obtain contact information and continue with personal outreach to assist with career development (scholarship opportunities/introductions to leadership/mentorship) to eventually be a paying dues member of chapter
Achievable
Goals need to be realistic to achieve them.
Relevant
Goals should be aligned with the mission, goals and pillars to success.

Goals should have a deadline. This doesn’t mean that all the work is done, but you can evaluate the success at said date to check off and/or reevaluate.
